2)) Is there a theme to your layout?
(ex: flowers, hello kitty, racing...etc)

3)) Do you want a matching contact box, online indicator, and/or in your extended network image?
4)) Where do you want your layout delivered to?
(either your myspace site or email)

5)) Did you add me on my myspace site?
