| Registration for the 2004 Prairie State Tandem Rally is $6.00 per person, adults and children. We pay for insurance with the registration fee so we must receive a fee for anyone who will be on your bike. This year's t-shirts will have the Rollin' Down the River logo on a yellow t-shirt. Tee shirts are not available after May 19, so get your registration in early. . Please make your check payable to the Prairie State Tandem Rally and mail it with the completed registration, waiver, and emergency contacts to: Paula Myszczak 2004 Prairie State Tandem Rally 75 Washington Boulevard Hoffman Estates, IL 60194 If you would like a snail mail confirmation, you must include a self-addressed, stamped envelope with your registration. Otherwise, be sure to include your email address on the registration and you will receive confirmation via email. ATTENTION PARENTS: Registration fees are used to pay for insurance and other administrative costs. A fee must be submitted for anyone who will be riding in the Rally, regardless of age. Any registrations received without fees for participating children will not be processed until all fees are received. Thank you for your understanding and cooperation. Fees increase and tee shirts are not availble if registration is received after May 19, 2004, so don't delay. |
