Day Case Surgery

Definition:

 A patient who is admitted for investigation or operation on a planned, non-resident basis and who requires facilities for recovery.these includes:

Ø      Full admission

Ø      Full discharge

Ø      Trained staff and equipment

 

Objectives:  Why Day case surgery?

1.      Reduction in cost

2.      Minimal disruption to patients normal routines

3.      Psychological benefit

4.      Low post op morbidity (reduced risk of infection)

5.      Waiting list clearance

 

Selection Criteria:

Ø      Physical status: class I or II ASA. (American society of Anaesthesiologist)  Either healthy or with medical problem that is well under control.

Ø      Obesity: body mass\height more than 34% requires admission to avoid increase morbidity.

Ø      Age: not too young nor too old      <1year should be admitted. Physiological age more important than chronological age.

Ø      Operation: bleeding, post op pain, more than 2 hours.

 

Pre operative Screening:

1.      Health questionnaire

2.      Investigation

3.      Fasting ( 4 hours for food, two hours for liquids)

4.        Premeditation.

 

Discharge Criteria:

1.      Alert, oriented

2.      Tolerating food and liquids

3.      Lives at less than one hours drive from hospital

4.      Facilities for toilet at ground floor in first 24 hours

5.      Accompanied by a responsible adult.

6.      Detailed instructions for patient \GP  regarding

Ø      Procedure done

Ø      Wound care

Ø      Drain care

Ø      Removal of stiches, drain.

Ø      Pain control.
