Health Requirements for 7th Grade

Sports Physicals

All sports physicals must be dated NO EARLIER then August 1^st every school year.

Requirements BEFORE the first day of school for Grade 7

_____  Physical Examination – done within one year prior to the first day of

school (August 30, 2004 to August 30, 2005)  

            _____  Td booster – tetanus/diphtheria vaccination within the last 5 years (2000 to 2005)

            _____  Hepatitis B Series – series of 3 immunizations

            _____  Varicella vaccination – one if before thirteen years old and 2 if first is

after age 13, or physician documentation of having had chickenpox

            _____  Measles, Mumps, and  Rubella vaccination- 2 doses required         

            _____  Dental visit within the past 6 months (March 2005 to September 2005)

Scoliosis Screening

All students are required to be screened for scoliosis annually in grades 6, 7, and 8. If your child has been screened by their physician the RESULTS must be sent into the school. Students will be educated about scoliosis and will receive notification of any upcoming screenings at least one week in advance of the screening.
