Enrollment Procedures and Checklist

 

 

 

There is a registration fee of $25.00 per family. This non-refundable fee is to hold your child's spot as well as cover the cost of paperwork required to enroll your child. This fee does not apply toward child care. In the event your child does not start on the agreed upon date, your position will be forfeited.

 

There are many forms that must be completed and handed in before I can assume the responsibility of caring for your child. (There are NO EXCEPTIONS. Most of these forms are required by the State.)  You will also be required to pay the first week’s fee when dropping off enrollment forms.

Fee Schedule and Holidays

 

We operate a 24-hour facility covering 11 hours of care per shift. Our week runs from Sunday through Saturday.  Any care needed after 11 hours will incur the additional charges as stated below. 

 

FEE SCHEDULE: 

 

Registration Fee:  $25 (non-refundable)

§         Full-time care entitles you to five days of care per week for your child whether they attend or not. An additional daily charge will incur for subsequent days.

§         Part-time care (up to three days) is billed at the daily rate whether they attend or not.

§         Your daily fee entitles you to care for that day only and must be paid upon drop-off.

§         Fees cover all basic supplies and food.  Parents of infants must supply diapers and formula.

§         All fees are due on Friday unless other arrangements are agreed upon in writing. You will be terminated if fees go beyond two (2) weeks.

§         Financial statements detailing total amount paid will be issued along with a W-10 each January or within one month after termination.

 

LATE FEES:

 

An immediate late fee of $15 will be assessed for children picked up after their scheduled time.  An additional $5 for every fifteen (15) minutes or portion thereof will be added to the original late fee.  This fee must be paid at pick up or before care the next day.

Vacations & Leaves

 

 

VACATIONS:

 

Each family will be allowed a one (1) week vacation annually without pay.  You must give two (2) weeks notice before taking your vacation and it may not coincide with my vacation week.

 

The day care will be closed two weeks per year for my family vacation. These are paid vacations.

 

 

PERSONAL/SICK LEAVE:

 

Since my husband and I both run the daycare, the chance of both of us being unable to provide care for the day is minimal.  Therefore, unless there is a serious illness that requires one of us to be available for the other (emergency situations or serious surgery), we will remain open and use our assistants to maintain mandated coverage. 

 

In the event of an emergency, my daughter, Katrina Lewis, who is also registered with my daycare to provide care, along with my son, Kevon Lewis, will be asked to come in and oversee operations during our absence.

 

 

FUNERAL LEAVE:

 

In the event of a death in my immediate family (husband, mother, mother-in-law, father-in-law, sibling, child, grandchild), the daycare will be closed for five (5) days from the date of death.  Parents will be required to make alternate arrangements during this time of bereavement. 

 

 

Dress Code

 

 

All children are to be dressed and clean before arrival. Also, send them in PLAY clothes. We may do messy activities and although I try to keep them clean, they are kids and may end up dirty! Do not send them in anything you do not wish to have soiled either w/ washable paint, dirt/sand from outside, grass stain, etc., because I will not exclude a child from an activity because of what they are wearing!

 

I am NOT responsible for any clothes or the cost of replacing such items. 

 

 


 

House Rules

 

Children

 

1)      No biting, hitting, pushing, shoving, or any undesirable physical contact.  Please respect the space of others.

 

2)      Name-calling intentionally to hurt another’s feelings is not allowed.  Please treat others as you like them to treat you.

 

3)      No running or rough-housing in my home.  This can be done outside.  Bedrooms in my  home are off-limits to daycare children.  Male school-aged children may be allowed in a  pre-designated bedroom at my discretion.  Respect for my home is mandatory.

 

4)      No picking up other children, especially infants and toddlers.

 

Parents

 

1)      No food or drink is to be brought from home at arrival or departure time.  Please allow adequate time for your child to finish eating before arriving at daycare.

 

2)      Property brought to child care is not my responsibility. I cannot be responsible for lost or broken items. I do not prohibit children from bringing toys, movies, games, etc. from home, but they must be safe (and appropriate) for all ages. If the children are unable to share, the item(s) will be put away for the day. Any property that remains at Mom-Mom’s Place FLC ten (10) business days after last day of child's attendance becomes the property of Mom-Mom’s Place FLC to be disposed of as I see fit.  Please empty your child's cubby on their last day of attendance.

 

3)      Proper use of day care toys and equipment is required at all times for safety reasons and maintenance.  If a child deliberately breaks a toy, the parent will be responsible for replacing the item.

 

4)      Nap/quiet time is required every day for children in my care.

 

5)      Please treat my home and family with respect.

 

6)      Please treat other families and children who attend my daycare with respect.

 

7)      Smoking is NEVER allowed in my home.  Please do not throw your butts in my yard or on my porch and exhale before entering my home.

 

8)      You are welcome to visit your child at any time.

 

9)      Parents may call the center at any time, however, if I’m unavailable, I will return your call within two (2) hours.


 

Health Policies and Procedures

 

First-aid procedures

§         Red Cross first-aid and poison center guidelines will be followed.

§         All providers are certified in emergency care and CPR for children and infants.

§         A first-aid kit containing the necessary items as identified by the Health Department is kept on the premises.

§         Emergency numbers are posted at all times.

§         Parents will be notified of accidents.  An injury record is kept for each child.

 

Medical emergencies

§         Immediate first aid will be given.

§         Ambulance will be called and paramedic recommendations followed.

§         Parents will be notified as soon as possible.

§         When parents are not available, your emergency contact will be called.

§         Parents are responsible for any expenses as a result of emergency room care, ambulance, etc.

§         Children’s Hospital of Philadelphia will be used for emergency care.

 

Illness

§         If your child becomes ill during the day, you will be notified and expected to pick up your child as soon as possible.

§         If your child will be missing day care due to illness, please notify me by phone the previous evening or the morning of the day of the absence.

 

Communicable disease prevention and reporting

§         Your child must be current in his/her immunizations.

§         Your child will be kept isolated from other children as much as possible if your child has a communicable disease.  Communicable diseases will be reported to the Department of Health.

§         Each child will be observed daily for illness.

§         Individual bedding, cups, sippy cups, bottles and utensils will be used to minimize the spread of infection.

 

Handwashing, diapering, sanitizing, and laundering policies

§         Child care provider will wash hands before and after diapering, toileting, eating, handling and preparing food, and handling contaminated materials.

§         Diapers will be changed regularly and immediately after a bowel movement.  Bottoms will be thoroughly cleaned and petroleum jelly will be used as needed.  Other types of preparations will be provided by parents.

§         The diapering area will be sanitized after each use, and contaminated utensils/toys will be sanitized.

§         Bedding will be laundered weekly and kept on premises.

§         Bottles and nipples will be washed in hot water and rinsed well before filling.

 

Management of medication

§         All prescribed medication must be labeled with the child’s name and must be accompanied by written permission from the parent for dispensing.  Medications must be in the original containers as prescribed.

 

Health records

§         Immunization records and the date of your child’s last physical examination must be kept up to date.

§         Please notify me of any special health problems or concerns regarding your child or family.

 


 Environmental Safety Policies

 

The following safety measures are taken to provide a safe learning environment for the children.  Most importantly, children are continually supervised, and the home maintains the proper staff/child ratio as set out by the State of Pennsylvania licensing office.  Also,

 

§         Emergency numbers are posted by the phone.

§         Cribs, high chairs, and other equipment are safe and work well.

§         Small or broken toys are removed.

§         Electrical outlets have socket covers.

§         Furniture and equipment are maintained to ensure safety.

Things that can cause harm to children are kept out of reach:

 

§         No cords or tablecloths are hanging.

§         Knives, razors, matches, cleaning supplies, medicines or other toxic substances are locked in cabinets

§         Cooking is done on the back of the stove and children are not permitted near stove.

§         No guns in the house.

 

Incident Reporting Policy

 

When a serious incident involves a child in this family child care, an incident report is made.  This information is provided to the parent/guardian, licensing board and insurance company.  The purpose of these reports is to inform and follow up on actions taken concerning the incident.  Incident reports are made for the following serious incidents:

 

§         Injuries involving serious accidents, falls (2 feet or more) or motor vehicles

§         Unexpected illness, contagious disease

§         Poisoning or medication error

§         Aggressive or unusual behavior

 

In addition, the child care provider is required by law to report any observed or suspected child abuse or neglect to the Department of Human Services.


 

Fire Safety Policy and Procedure

 

 

To reduce the threat of fire, smoke detectors are installed in each room and checked regularly to make sure they are in working order.  Fire drills will be conducted every other month.

 

In case of fire, the children will evacuate the day care from one of two doors – front door or back door.  Parents will be called immediately.

 

 

 



 

Nutrition Policy

 

Mom-Mom’s Place FLC believes in providing nutritious meals for children.  We follow the USDA food program guidelines and ensure the nutritional quality of meals and promote healthy eating.  In order to participate in the program, meals must include the following nutritional components.

 

Breakfast

 

§         Milk

§         Fruit or vegetable or 100% juice

§         Bread or cereal

 

Lunch

 

§         Milk

§         Meat or meat alternate

§         Fruit or vegetable (2 servings)

§         Bread or alternate

 

Snacks

 

At least 2 of the 4 components

 

§         Milk

§         Meat or alternate

§         Fruit or vegetable

§         Bread or alternate

 

Food is always served in kid-friendly portions.

 

 

If your child misses a scheduled meal, please make sure they eat before they arrive.

 

 


 

Toilet Training Policy

 

Please let me know when you feel your child is ready to begin learning to use the potty. This is your responsibility but I will assist you in any way possible.  Please advise me as to the terms that you would like used during toilet training.  Please understand that your child’s learning experiences will not be a private matter.  I am responsible for all of the children in my care and must be aware of the other children’s whereabouts at all times.  You will need to provide me with changes of outfits and undergarments.  Soiled clothing will be sent home for laundering.  Please make sure your child wears clothes that he/she can remove without help (NO onesies, belts, snapped pants, etc.)  I will never discipline nor belittle a child who has an accident.  This time should always be a positive experience.

 

 

 

Transportation and Field Trip Policy

 

Once a month we will have a field trip for the day care.  These trips usually require additional funds and the children will be transported in a safe vehicle with proper insurance and licensing.  You will always be notified of a field trip and asked to sign a permission slip.

 

Every week (except in inclement/cold weather) we will take walks in the neighborhood to the playground, etc.  We will ensure sufficient staff coverage for these outings.

 

 

 

 

 


 

Discharge Policy

 

Child Withdrawal

 

If the parent/guardian finds it necessary to withdraw a child from care, he/she must give a two (2) week notice prior to withdrawal.  All fees must be paid in full in cash prior to final date of attendance.  Parents not providing written notification shall be liable for charges including, but not limited to: 2 weeks tuition, late fees for nonpayment and any other fees that have been incurred with the daycare. Parent(s) will be liable for any and all collection costs in connection with any outstanding tuition.

 

If your childcare account remains unpaid for any reason, be advised that your account will be reported to ProviderWatch. ProviderWatch is a credit-reporting agency that specializes in childcare accounts. Your delinquent account being reported to ProviderWatch will likely make it more difficult for you to find childcare providers willing to accept your children until any such accounts have been paid. You may contact ProviderWatch if any childcare provider informs you that their decision not to accept your child into care is based in whole or in part on information received from this agency. ProviderWatch will disclose any delinquent account information on record so that you may resolve these accounts.

Child Discharge

 

It might be necessary for me to give notice to a parent/guardian to stop caring for a child.  I will give the parent/guardian two (2) weeks notice prior to ceasing care.  In rare cases of severe problems, I may be forced to release the child from care without giving a two-week notice.  Care may be terminated for the following reasons:

 

§         Child is increasingly unhappy and unable to become comfortable with the family child care setting

 

§         Parent/guardian routinely abuses drop-off and pick-up times

 

§         Parent/guardian doesn’t pay child care fees on time

 

§         Parent/guardian does not work with the provider as a team to provide consistent guidance and discipline.
