Your child's transition starts at age 14, and continues until graduation. When your child turns 14, his/her special education teacher should start preparing an individualized transition plan (ITP) to prepare for their life after high school. The ITP concerns your child's hopes and dreams for the future and gives them the knowledge to achieve these dreams.. The ITP deals with social skills, job skills, community based instruction and many other areas which help prepare your child for their future. It starts off broad then narrows down to specific goals and objectives designed to get you child ready for higher education, a job, or independent living. The ITP is a group process, which you, as a parent, must be a part. A ITP planning team meets every year to design and update your child's ITP. The special education teacher, regular education teacher, the student, a school representative, a local agency representative (in your child's senior year) and you are the members of this team. You know you child the best - use this knowledge to guide your child in the right direction towards independence and a fulfilling life. |
