Emergency Care Report

12:15 Dan went under water
12:20 Dan was recovered from beneath the water
Single person CPR begun
Dan was not breathing and had no pulse ( dead on scene)
He was cold, clammy, pale skin, blue lips, etc…
Head to toe scan showed no sign of external injury
C spine immobilization was implemented immediately
Dan was kept in an anatomical position
12:23 Two person CPR administered
12:27 Dan’s pulse returns along with shallow, snoring breathing
He was placed in recovery position with c spine maintained
He aspirated blood and vomit
He registered P on AVPU scale, responding to sternum rubs
12:30 Began recording sets of vitals (pulse and respiration)
Dan’s legs were elevated for 15 min.
Dan was completely removed from rising water, covered and shaded to reduce the risk of hypothermia,
heat exhaustion and shock
He was kept cool with water dripped on face and body
Shortly after breathing resumed:
Capillary refill normal
Lips regained color
Pupils were equal and reactive to light
Skin was flushed and clammy
He was rotated to a position head/feet
C-spine and recovery position maintained
More blood aspirated
Dan goes into anaphylactic shock
V on AVPU scale
Shock escalates and maintains intensity throughout care given at site
Characterized by teeth grinding, eyes opening, flexing arms, legs, convulsing, vocalizing pain
Nasal passage become clogged but was emitted with breathing
Dan was given jaw thrust maneuver to maintain an open airway
2:30 Helicopter circling area
3:00 Helicopter landed
Doctor administered 95% oxygen at rate of 12 by means of non-rebreather mask
Fluids were administered by I.V.
Dan was floated across water with oxygen and I.V. on
He was carried 100 yards to a suitable landing site
3:20 Oxygen ran out (20 minute supply), wait for helicopter to mobilize
Dan is loaded on, long spine board, secured
3:45 Helicopter leaves site with Dan
