Parental Consent and Release
I do hereby give permission for my son/daughter _________________ to attend Seminar Hadracha on Octobet 24 through October 26, 2003.

By signing below, I, for myself, my son/daughter and my heirs, representatives and next of kin release and discharge Greater Midwest Young Judaea, Young Judaea, Hadassah Zionist Youth Commission, Hadassah- the Women's Zionist Organization of America, Inc., and Hadassah Medical Relief Association, Inc. and each of their affiliated entities, officers, directors, employees, volunteers, members and agents, from and for any and all claims, demands, actions, rights of action and liability of any kind whatsoever, whether in law or in equity, that may be incurred by us or which we may have, arising from, related to, or in connection with, my son's/daughter's participation in the event, including but not limited to liability for claims of bodily injury, death, illness, disease or property or other damage my son/daughter may suffer.

In case of medical emergency, I understand that every effort will be made to contact parents or guardians of convention participants immediately. In the event that I cannot be reached, I hereby give permission to the physician seleted by the convention director to secure necessary and appropriate treatment for my child including hospitalization, anesthisia, surgery and any other emergency treatment for my child deemed necessary by the physician for my child as named above. I understand that Young Judaea and Hadassah are not liable for any medical expenses incurred by my child while attending the event and authorize contact and dealing with my health insurance company directly, as necessary, as enumerated in this application.

I agree that this agreement is to be governed by, and constructed in accordance with, the laws of the State of New York, without giving effect to the principles of conflicts of law.

Convention/Event Rules: Participant and Parent Must Sign
1. Smoking is absolutely prohibited, regardless of participant's age. There are no          exceptions.
2. In order to provide an atmosphere which is comfortable for everyone, Shabbat and  Kashrut will be observed.
3. No one may leave the grounds without specific permission of the Regional Director.
4. All participants must respect the property of each other and of the convention site.
5. All participants will attend the entire convention, including all of its schedules          activities, unless otherwisse informed by the staff, it being understood that physical      sports are always optional.
6. Any damage caused by child will become the parent's responsibility. Please be  advised that anyone who damages camp property, will be billed for the                      damages INCLUDING a MINIMUMof $50, plus materials and labor, for                    defacing (writing on or otherwise defacing) any camp property.

I have read and understand the Convention/Event Rules, and will adhere to them. I also understand that violation of these rules my result in expulsion from current and future Young Judaea activities.
