For and in consideration of MASC acceptance of this application, I hereby swear, certify and/or affirm that the information given by me is truthful and complete. I further certify that I am fully aware of the activities involved in the GSDCA-WDA National Schutzhund Championship & World Qualification Trial and as such I am cognizant that injury is a distinct possibility for my dog or me personally and with that knowledge, I hereby fully indemnify and forever hold harmless, MASC, the GSDCA, the GSDCA-WDA, their officers and directors and members as well as the property owners on whose land the event is held from any injury to me, my dog, my family, or property that may be alleged, implied or real and resulting from or as may be alleged to have resulted from my participation in, or while traveling to and from, this event. I further accept full and complete responsibility for the actions of myself, my dog, family, and handler(s), and indemnify and defend MASC, the GSDCA, the GSDCA-WDA, and/or the property owners on whose land the event is held for any and all claims of injury or damages as may be alleged or otherwise. Said indemnification and defense include, but not limited to, attorney fees and those expenses involved in providing adequate defense against any and all claims whether alleged or proven at law or equity. I fully understand and agree that MASC may remove and/or dismiss me, my dog, family, or handler(s), from the event at their sole discretion if in their opinion, I have violated any rule or should I, my dog, family, or handler(s) act in an unsportsmanlike manner or pose any threat to the tranquility, safety, and/or well being of others at the event. I also understand and agree that all funds submitted to MASC are earned in full upon receipt and not subject to refund either partially or in full. I further agree to be under the control of the officers and directors of MASC, judges and all event officials during the event and acknowledge that their decisions are final and may not be appealed before any organization or authority at law or equity.

Mail completed application form, copied recognized AKC Dog Registration, GSDCA-WDA Score Book, certified pedigree, copy of GSDCA membership card, and copy of GSDCA-WDA membership card along with the appropriate U.S. currency (made payable to MASC) to:
