Small yellow Lady-slipper
Cypripedium calceolus var.
                          parfviflorum
The individuals of this orchid species are highly variable, with many different varieties recognized of the same species.  This one is the Small Yellow Lady's Slipper.  The species name derives from Latin and means "a little shoe."  Cherokee Indians made a preparation from the roots that was drunk as a treatment for worms.
