After meeting with Artemis, she is the first to become a Sailor Scout.  As she is slowly dying of what appears to be a brain tumor-and refuses a surgery that could save her, as the chances of survival are low and she does not want to risk leaving this Earth any sooner,- she is happy to have a purpose in life.  She becomes very serious and vows to complete the mission from her past life, originally fighting crime as Sailor V.  She regains her memories after battling with Tuxedo Mask.  She is an idol in both her forms to Usagi and her friends and also Makoto.  Singing is the only joy she allows herself.  Most of her songs and album titles somehow hint at her secret identity.  Recognizes Usagi as Sailor Moon and Princess Serenity the minute she sees her.  As the leader of the princess' guardians, she takes up the role as decoy-making everyone believe she is the princess and has the Silver Crystal.  She tries to help the other Sailors awaken their powers, even forming a friendly but fierce rivalry with Rei.  Her headaches and fainting continue to get worse, and she realizes time is getting short.  Rei learns of her identity and her illness, and agrees with Artemis that she should value her own life enough to have the surgery.  She gets mad at both of them.  She does realize that she is truly scared of what will happen to her though.  Her plan to quit singing and devote all her time to her mission is foiled by the appearance of a rival, Mars Reiko-Rei.  When she still refuses to have the surgery, Rei reveals that Minako has not awakened her own powers yet and says she is not fit to be leader.  Then, Makoto attempts to sacrifice her own life to stop a youma.  Minako asks her in the hospital why she did such a foolish thing, and is taken aback to learn the act was inspired by her own decision to risk everything for her mission.  Then her illness stops her from being able to transform, so she finally decides to try the surgery.  Tragically, she dies-apparently before even getting the operation.  Her spirit consoles the upset Scouts, and her weapon is wielded by Mars.  She is revived in full health when the world is restored by Usagi and Mamoru, but has no memories of her Scout life.  Her memory returns along with them, and the girls are reunited.  In the Special Act, she is in London and has just become the first Japanese person to receive a prestigious entertainment award.  She returns to Japan to fight the evil that appears there, wielding the sword that restored the Scouts powers.  After witnessing Mamoru and Usagi's wedding, she returns to her idol life.
