medication error, the Incident/Occurrence Report is the 1st line of defense
against a medical liability claim.
- The Occurrence
Report should :
- Contain complete &
appropriate information.
- Be legible to prevent
misinterpretation.
- Never contain
personal or professional conclusions, opinions, accusations, criticisms, or
admissions of guilt.
