  • If you make a medication error, the Incident/Occurrence Report is the 1st line of defense against a medical liability claim.
  • The Occurrence Report should :
    • Contain complete & appropriate information.
    • Be legible to prevent misinterpretation.
  • Never contain personal or professional conclusions, opinions, accusations, criticisms, or admissions of guilt.
