 Causes of Medication Errors

  • Lack of knowledge about the drug.

  • Failure to follow the hospital policies /procedures.

  • Faulty drug identity.

  • Incorrect use of infusion pumps & parenteral delivery systems

  • Inadequate patient monitoring

  • Transcription errors.

  • Illegible or incomplete prescriptive orders.

  • Faulty dose checking.

  • Lack of patient information.

  • Chaotic work environment.

  • Verbal orders.

  • Borrowing drugs from another cart

  • Incompetent or impaired personnel.

  • Lack of dosage or interaction checks for high-risk drugs ,e.g., coumadin, Plavix, & ASA ordered for same patient.

  • Math errors, e.g., 12/4=4.

  • Defective or similar packaging or labeling..

  • Use of multidose vials.

  • IV solutions not premixed.

  • Names of medications that sound alike & look-alike, e.g., Lamical or Lamisil; Celexa or Celebrex; Zyprexa or Zyrtec.
