Close Window Restraint Order Procedures
  • MD provides verbal OR written order within ONE HOUR after restraint initiated
  • MD evaluates patient in person within ONE HOUR of initiation of restraints.
  • MD must re-evaluate need for restraint use AFTER FOUR HOURS
  • Notify family or legal representative of restraint use.

 

Patient Care

  • Trial release and re-restraint allowed within 4 HOUR period
  • Continuous monitoring required
  • Assessment EVERY 15 MINUTES
  • Restraints release at 2 HOURS for PROM

 

Documentation Requirements

  • Document care provided and monitoring Q15minutes on restraint flow sheet
  • Restraint progress note on initiation of restraints
  • Complete restraint flow sheet with D/C of restraints
  • Document criteria used for removal of restraints
  • Physician written de-briefing notes
