As an employee, you might be eligible for
coverage under the Workers’ Compensation Act of your state which provides medical
benefits and compensation for time lost from work. Here are the steps you
should follow if you want to be covered under the workers’ compensation laws in
your state:

Although
you may have been injured at work, a claim can be considered compensable under
workers’ compensation laws only if the injury occurred by accident and is
arising out of and in the course of employment.

Tell your manager immediately after
you have been injured or exposed to a disease, even if the injury seems
minor. By telling your manager as soon as possible, you will protect your
rights under the law.

If you require emergency treatment, go to
any emergency room. Workers’ Compensation will cover all medical expenses
related to an initial emergency room visit.

Complete the appropriate reporting form
within 24 hours. (XXXXX Hospital employees complete the “Accident Report of
Workers’ Compensation Claim”; XXXXX Physicians employees contact their
supervisor for appropriate forms.)

Select a physician to treat your
illness/injury from the employer’s panel of physicians. If you go to your own
physician, you may waive rights to workers’ compensation.

Take completed forms to the appropriate
office:

All claims for workers’ compensation are
reviewed to determine if your claim will be covered under the provisions of
the Workers’ Compensation Act.
