Reporting an Incident
Before you report an incident, please try to have as many details about the incident as possible.
We need to be able to prepare, as precicely as possible, for the event we're responding to.  Our safety depends many times upon being able to prepare for as much as we are able to prepare for, before we arrive on the scene.  Once we're there, it's too late to prepare and we may not have time to leave the scene to go get an instrument or safety device that we should have had, had we known beforehand that we may need it.
We know that situations of this nature can change fast, and can go very quickly from being stable, to dangerous.   Being as prepared as possible lessens our personal danger to a great degree, but we cannot prepare for something that we don't know about.  Therefore, we're going to want to know exactly what happened, how, who was involved, how serious the incident is, the location of the incident, and what took place-just about everything and anything you can tell us regarding the event will be useful, no matter how trivial it may seem.  Our personal safety may rely upon your giving us information that is as complete as possible.
We understand that some events may defy proper explanation; all we ask is that you do the best that you can.

We thank you, and  we appreciate your willingness to inform us of this event.
Be assured that we will respond as quickly as possible.
