First, some special notes about how forms work, adapted from page 190 of Creating Web Pages Simplified, published by IDG Books in 1996:

After viewers of your website complete your forms, they select a "Submit" button. Their information is then transferred to your site's Web server. When the Web server receives the information from a form, the server runs a program called a Common Gateway Interface (CGI). CGI scripts convert the submitted information into details the owner can understand. Before you include forms in your website, be sure that your Web server can run CGI Scripts.
