is to acknowledge that I/we have received and read the Light of Christ Parish
Religious Education Handbook. I/we understand and agree to cooperate with the
policies set forth in the handbook.

addition, I/we have received, read, understand, and accept the Harassment
Policy in Non-Employment Situations regarding the schools, early childhood
centers, parish youth ministry programs, par9sh religious education programs,
Boy Scouts and Our Lady of Good Counsel Camp of the Diocese of St.

Also, I/we acknowledge that from time to time, publicity for
a newspaper, television station or other media may be prepared about parish
events. These may or may not be accompanied by photographs or videotapes of
students. I/we give permission for publicity releases prepared by Light of
Christ Parish or media representatives.
