The purpose of EVOC 503 WA2 was to prepare a Work Study Contract for one of the students in my Information Technology courses that I teach. The following six criteria were specified:

  1. Comply with all of the General Criteria for Written Assignments.
     

  2. Develop an "Work Experience Contract" for a particular student. The contract should not be more than 1 page (typewritten or word processed).
     

  3. Include specific information in the contract about what the STUDENT is expected to do.
     

  4. Include specific information in the contract about what the EMPLOYER is expected to do.
     

  5. Include specific information in the contract about what the INSTRUCTOR is expected to do.
     

  6. On the cover memo (see #1 above) indicate and briefly describe at least one specific resource from which additional information about work study programs can be obtained.

 

The following sources of information were used relative to this assignment:

WORK STUDY CONTRACT

The A.V.U.H.S.D. recognizes that the primary purpose of Career Technical Programs is to provide students with the kinds of training experiences that will enable them to make better career decisions, and develop vocational skills.

TEACHER, SCHOOL RESPONSIBILITIES
As a teacher/coordinator in the Community Class Program, I agree to:

  1. Inform the student of program rules and regulations.

  2. Provide student with necessary program forms.

  3. Instruct the student regarding his/her dual responsibility to the training site and to the school.

  4. Coordinate the student’s on-the-job training experiences with his/her in-school related learning activities.

  5. Work closely with the training site in order to provide the student opportunities to benefit from his/her training experience.

  6. Visit the student’s training site and consult with the supervisor regarding the student’s performance.

  7. Assist the student to resolve school or training related problems that are affecting his/her training performance.

  8. Provide the training site with evaluation forms, and discuss the student’s training performance with the supervisor and the student.

  9. Perform a liaison and coordinating function between the school and the business/industry/community.

  10. Sign the training agreement that indicates acceptance of the student in the program.

STUDENT, PARENT/GUARDIAN RESPONSIBILITIES
As a student and parent/guardian of a student in the Community Class Program, we agree to:

Follow the program rules and regulations established by the school and the training site.

Submit a time card weekly as required by the teacher to verify the number of hours trained.

Not receive credit for time trained during week of absence from class.

Notify the teacher AND the training site of school or training related problems that may affect their performance.

Maintain regular learning activities, attendance, and punctuality in school and on the training site.

Fulfill related learning activities and attend meetings as required by the teacher.

Not be paid while training in community classroom or at the community classroom training site.

Not be guaranteed a job on completion of training.

Be responsible for own transportation and parent/guardian must accept liability if incurred.

Dishonesty at the training site, altered or fictitious contracts or time cards will result in a failing grade and a loss of credit.

1.  The school will not authorize, nor be held responsible for the mode of transportation used.
2.  The Antelope Valley Union High School District’s attendance policy or the equivalent will apply.
3.  Signatures indicate an understanding of the responsibilities and acceptance of the program’s requirements.

TRAINING SITE RESPONSIBILITIES
As a training site sponsor in the Community Class Program, I agree to:

  1. Inform the student of rules, regulations, and functions expected of him/her.

  2. Ensure that the training site conditions shall not endanger the health, safety, welfare, or morals of the student.

  3. Supervise the student; assist him/her to improve his/her training performance.

  4. Assign him/her opportunities to learn by practicing, enhancing, developing and/or observing skills taught in the related classroom as stated in Title V of the Education Code.

  5. Assure that no discrimination will occur on the basis of race, creed, color, gender, religion, or handicap.

  6. Abide by State and Federal laws/regulations pertaining to employment.

  7. Students may not be used to replace regular employees or cause their hours to be reduced.

  8. Verify the hours the student trained by signing his/her time card weekly.

  9. Discuss student performance with teacher and complete evaluation.

  10. Inform the teacher when planning to terminate a student’s training.

  11. Sign the business industry agreement that indicates a willingness to participate as a training sponsor, and to accept the program responsibilties .
