| The following are questions to be answered to the best of your ablilty. Do not skip any qestion. All Must be answered fully and honestly. Answers such as " I dont know, I am not sure" etc WILL NOT BE ACCEPTED. |
| 1. Do you have any medical difficulties your Mistress needs to be informed of? ( example: asthma, high blood pressure) 2. Do you have any phobias your Mistress needs to be informed of? ( example: claustrophobia ) 3. Do you have a history of clinical depression or any type of psycholigical instability? 4. Are you drug free? 5. Are you disease free? 6. Do you pratice safe sex? 7. You have been ordered to perform a task that you dislike. What should you do? 8. You have been ordered to complete a task and for some reason you were not able to complete it. What should you do? 9. Its 6am and you have been ordered to make and serve your Mistress coffee, but you are still tired and wish to go back to sleep. What should you do? 10. Do you understand the difference of punishment for misbehavior or displeasing your Mistress compared to the gift of pain you receive during play? 11. Your Mistress whipped you pretty hard the nite before and now She has called upon your services again. You are still sore form the play before and you are pretty sure your Mistress will whip you again. What do you do? 12.
