The dice game

Now I can’t recall how to play this game. I have a napkin, on which I wrote the rules, but now I cannot make heads nor tails out of my writing (likely because I was intoxicated at the time I was writing everything down.) I do, however, have my dice and the cup that they were thrown from. Just don’t have a clue as to what to do.
