Airline travel is less complicated than it was some three years ago after 9/11. Many of the security checkpoints have eased up and many of the prohibited carry-on items are now allowed. Still, allow ample time to get through security. Most airfare bargains can be booked directly through an airlines on-line page such as Delta.com or NWA.com Most tickets are non-refundable unless additional fees have been paid, so take care to plan your return trip at least three to four weeks after your surgery date. Many US flights to Germany utilize Lufthansa Air for the return flight. I would not recommend booking through Lufthansa direct as their rates are very high.
I have also included links to obtaining a US passport. I found that my local US post office was very convenient for applying and turn around was approximately six weeks, however, I was told it could take as long as two months. If you are in need of a passport immediately, see the link for a 24 hour turnaround.
