Filling out the Registration Form should only take you a few minutes.
It is a basic registration format that asks your name(s), address, contacting information, and other related data.

Names: [Input you and your spouse's full first and last name. If you prefer, put you and your spouse's first initial, and full last name.]
Address: [Input your address. This is used for mailing extra information to you]
City and Postal Code: [Input your City and Postal Code]
Phone Numbers [Input your Home, Business, and Fax numbers. These serve as a quick way to contact you personally]

Due Date: [Input your delivery date]
Hospital where delivering: [Input the Hospital where you will be delivering your baby]
Doctor: [Input your doctor's name]
Series Registering for: [Input the series you are registering for]
**"Series Registering for" is the class session which you and your partner plan to attend.
ie.) Sept 10/11 or Oct. 1/2 2004.
We run classes the first weekend of every month. Clients register for only one weekend class. Please browse the "Taking Classes" section to learn more about Lamaze classes, the schedule, and fees.**
