Once you have downloaded the form and filled it out, please send your form and cheque, postmarked by June 15, 2009 to : La Cloche Art Show, P.O. Box 149, Whitefish Falls, ON, P0P 2H0. REGISTRATIONS WILL ALSO BE ACCEPTED ON JUNE 19 and 20 IF YOU HAVE NOT PRE-REGISTERED.
