As a condition of registration to play, the player must sign this form indicating that he acknowledges that the Whitby Men’s Slo-Pitch League does not insure players, umpires or any voluntary associates of the league and does not accept any liability for any injury, however caused.  By signing this form the player thereby accepts the aforementioned conditions and exonerates the Whitby Men’s Slo-Pitch League and any of its officers and executive from any claim whatsoever in this respect.  Signature also indicates that the player’s information is correct and that the player will abide by the Rules and Constitution of the Whitby Men’s Slo-Pitch League.
