Subject: Transfer of the Electricity Meters to the name of beneficiary

Dear Beneficiary/Member,

This is further with reference to the transfer of electricity meter in the name of the beneficiary. Due to non-receipt of all the 74 forms we had to take up the matter with UGVCL for giving special consideration and the same has now been accepted. Hence all beneficiaries/members who have already submitted the duly signed forms in this reference to the society and are interested in getting the electricity meter transferred in their name are hereby requested to submit the following documents on or before 31 st Jan 2009.

1. Photocopy of the share certificate (both sides), if collected from society office
2. Photocopy of Index form (in case sale deed has already been executed), if not submitted earlier.
3. Photocopy of “Possession-cum-Occupation letter” issued by CGEWHO, if not submitted earlier.
4. Demand draft in favour of U.G.V.C.L. payable at par at Ahmedabad, for the respective amount as given below: (Kindly write your name and flat no. on the reverse of the cheque/demand draft)

The society shall submit only those forms to UGVCL for which the above mentioned documents are received on or before 31 st January 2009. Later the members shall be required to do the same on their own. Hence all interested beneficiaries/members requested to submit the above documents or before 31 st January 2009.
