When "Press Start" appears on the screen (Title Screen) press Up(2), Down(2), Left, Right, Up, Down, and A.
All level portals should be open without having to collect the required amount of fairys.
99 Lives:

When "Press Start" appears on the screen (Title Screen) press Left, Right(3), Down, Up, Right, Up, and A.
You should have 99 lives when you begin playing.
