FIELDS. Please
attach a copy of your grade report(s) to this application. Applications may be
submitted up to 6 months after the academic term being applied for.

Scholar (highest GPA in chapter for the academic term applied for)
Outstanding
Academic Achievement Award (GPA 3.5 or better out of a 4.0 scale)

Certificate of Excellence in Academics (average GPA of 2.75 or better for all
members)
Chapter must submit
copies of official grade reports from ALL members for the term applied for.
Note: an
individual will receive either a Top Chapter Scholar award or an Outstanding
Academic Achievement award, but not both. Pass/fail courses or courses for
which no grade is given, i.e., cooperative work programs or internships, will
not be counted for purposes of computing grade point averages.
