Before acquiring a new puppy, you will need to think about how the puppy will fit into your lifestyle, home and family. This questionnaire is designed to help you make important decisions prior to purchasing a puppy and to assist us in placing the right puppy with you. Your responses not only will tell us what you're looking for in a puppy, but will enable us to evaluate puppy personality and abilities with the goal of finding a good match. (Children six and under can have a very difficult time with small puppies.
A parent or sitter will have to be constantly vigilant during the early
stages of socialization. Children are unable to command respect and the
puppy must be taught to behave around each younger child. Children must
also be taught how to handle and behave around a young puppy. This takes
additional time and energy. If you have children in this age range,
be sure you have plenty of extra time to work with pup and child(ren).
Young children should NEVER be left unattended with the puppy.)

into your budget? (Initial puppy shots, monthly heartworm medication,
annual booster vaccinations, crate, bedding, food, toys, grooming
products, spaying or neutering).   
Are you committed to caring for this dog for his/her lifetime? (Average
life span is 15 yrs.)

What type of outdoor facilities do you have? Securely fenced yard,
kennel run. Invisible fence. Dog house or other adequate shelter from
the elements? (Invisible fencing will keep your dog in, but will not
keep other dogs out.) 
Do you padlock your gates (If applicable)?   
Do you plan to use a crate for housebreaking?

Are you willing to obedience train this dog through training classes
in order that he/she will become an enjoyable companion and canine good citizen?

"All puppies sold as companion dogs require a mandatory spay/neuter
contract. No exceptions. AKC and UKC registrations papers will be
sent to you after the companion dogs have been spayed or neutered.
Puppies sold for show will go with AKC and UKC registration papers."
