(Fax: 020 7 801 2852) giving your name & the address you want to receive your ballot at. Explain that you haven't received your ballot paper & quote your membership number which you will find on your union card and also the membership database, or your National Insurance number. Please make sure you sign your fax. If there are any further problems please contact your local rep.
