- Fill the following information
listed below into the "Return
Service Form" to receive a Return Merchandise
Authorization (RMA) number
- Information needed for
"Return Service Form"
- Customer ID#
- Quantity
- Invoice# from the packing slip
- COMPU-CLUB Item#
- Invoice Date
- Serial# from the product
- State your reason for returning
the purchased product (defective, DOA, non-defective)
- If the product is DOA (Dead on
Arrival) we will give you a refund or a replacement
- Please state if the product has
been opened or not
