BirthDate is in MM/DD/YYYY format.

There is a textbox in right of the label "Powered by -Keyur". It is nothing but a password field.
Type in the password and only then select a person from the dropdown menu.
It will not work unless and untill u type in the password.
So if u dont know the password then You can ask me.
