List of Documents I Sent to NSC:
Form G-28:  Notice of Entry of Appearance as Attorney
This is a form you sign indicating that you have an attorney representing you.  You would need this if you choose to have an attorney represent you.

Two color visa photos of myself and two color visa photos of her.

Informal photos of us together;  three of which shows our faces clearly.
A copy of my birth certificate.  (optional if I furnish copy of my U.S. Passport)
A copy of fiances's passport (signature page, picture, page containing entry and exit      visa in the country in which I met her).  In our case, she visited me here in the
     USA which is why I copied her passport pages.
Proof of termination of my previous marriage(if any).
Proof of trip to country in which we met.  - airline tickets, passport with entry stamps,
     I-94 paper, receipts, and boarding passes.
G-325A for myself and G-325A for her.
Check in the amount of $110 (as of the time I sent in the petition).
List of Documents I Sent After NOA2
I-134 form signed and notarized.  I included total income from federal income tax 1040 for the year 2002(line 22)  on the "I derive an annual income of..." on the I-134.  I included personal property (car, furniture, appliance, etc - use an estimate), 401K and mutual funds, stocks, and the value of my home along with mortgage balance on subsequent lines of  the I-134. 

Original copy of employment verification - The letter must have company letterhead on it.  It stated my name, title, date of employment and current salary..  It was signed by a representative in the personnel office.

Original copy of account verification from my bank.  It stated when I opened the account and my current balance.

Copies of 2000, 2001, and 2002 federal income tax return.

Copies of last 12 months' bank statements.

Copies of my 401k statement, mutual fund statement, and a lease copy showing myself as a lessor of the townhome.  It was not asked, but I included that as an extra layer of proof of income & assets in case it is needed.

Online Case Status
While you wait for your 2nd NOA, you can look up the status of your case online by using your receipt number found on the 1st NOA.  Do not type in the hyphens, but do type in the capital letters at the beginning of your receipt number.  Example:  LIN0303012345.
