| HEALTH FORM LINKS | ||||||||||
| A current health form must be on file with the Troop for all camping activities. A copy of the health form must be on file at the Health Lodge for camping at Bartle. | ||||||||||
| Class 1 Health Form To be used by all Cub Scouts and Leaders attending Cub Scout Day Camp, Bear Resident Camp, Webelos Camp and Boy Scouts and Leaders (under 40 years of age). This form is completed annually. Class 2 Health Form To be used by all Scouts and Leaders (under 40 years of age) attending Boy Scout Camp. This form is good for 36 months. (When using the Class 2 Health Form you must also print off the Class 1 form, this now becomes a front/back document) Class 3 Health Form To be used by all Leaders (over 40 years of age) attending Boy Scout Camp and ALL Camp Staff Members. This form is completed annually. |
