Below are some pictures of the foot throttle and thumb brake that I am currently using.

Here is a picture of the foot throttle that I designed. It will probably take one more round of mods to get it perfected. It's very usable as it is. It needs some stops installed to prevent overstressing the throttle cables.

The thumb brake is made from the stock Sportster brake master cylinder with  modified lever and mount. 

The modified lever allows the master cylinder to mount at a 90 degree angle to the bars. The mount is fabricated from aluminum. I get very good response from the front brakes. The front wheel can be made to chirp very easily.  There are no permanent mods to the master cylinder, but a new brake lever will be needed to put it back stock.

Here is how it fits my prosthetic. Note the rounded top so the prosthetic can pull up and off the adapter if I need to.
