Head and shoulders, shot with a 150mm portrait lens.

Pose: Subjects head and shoulders at an angle to the camera, head tilted slightly.

Lighting: I used one main light (Novatron w/ soft box) set at full power four feet away from the subject (f/16). A bounce card was placed at the subjects left, opposite to the main light. Next to the bounce card, I used a hair light, set at half power, three feet from the subject at a high angle coming down towards his left shoulder.

Camera settings: f/16 @ 1/125^th, #238. I used my Mamiya 645AFD (Medium Format) camera.
