Birth Certificate Information
There is a Birth Certificate for every recorded birth that was reported in Florida.

In the adoption world we break down birth certificates into two groups:

Original Birth Certificate (Sealed with adoption file)
Ammended Birth Certificate (Issued with adoptive parents names)

You may purchase your ammended Birth certificate in two styles:

Diploma Style
Long Form or Book Form Style

What an adoptee needs to request is the Book Form Birth Certificate.  This will provide more information than the diploma style.  Now, this still will not contain identifying information of your birth family.

Note:  Vital Statistics does not like to issue the book form style and sometimes will question you as to why you want this version.  My answer is because it is MINE, it is that simple.
