| Florida Office of Vital Statistics: Major change as of April 2003 affecting birth Family members: As of this date, the state of Florida will no longer provide for the birth family to receive ANY information pertaining to the adoptee or the adoption. Prior to this date, the birth family was entitled to the county and state the adoption was finalized in. County of finalization is now only available to adult adoptees (18 years of age or older). If you are an adoptee born in Florida and adopted in another state, you may request in writing, the county and state your adoption was finalized in. There is a fee of $9 for this information. Where to write for county of finalization: Florida Department of Vital Statistics Office ATTN: Lorraine Kratz - Adoption Unit P O Box 210 Jacksonville, Florida 32231-0042 The request should include the following: Adoptee Name (AN) Adoptee Date of Birth (DOB) Adoptee Place of Birth (POB) Adoptee Gender (male or female) Adoptive Parents Full name (AP) You must also include: A check or money order in the amount of $9 (make it out to "Florida Department of Vital Statistics" A copy of driver license or notorized letter OBTAINING YOUR ORIGINAL BIRTH CERTIFICATE: An adoptee may obtain their Original birth certificate ONLY after they have found or been found and reunited with birth family. To obtain your original birth certificate you MUST send signed and notorized affidavits from all parties, or provide death certificates for those that have deceased. You must also include the cost of your birth certificate, this changes in price so I would call prior to sending money. Your original birth certificate fee is the same as any other birth certificate fee.
