STUDENT PERFORMANCE WORKSHOPS, PLAYS EVENTS
CONTRACT
NEEDS
>  First name tags for participating students (for ease of working with students)
>  Number of students participating (for script copies)
>  List of students' full names and staff who participated (for each participant's     official and personalized "RECOGNITION CERTIFICATE"
>  To be paid in the timely and respectful manner as stated on this contract and     on the booking form

SUPPLIES PROVIDED
> One or two Student Workshop Performance facilitators
>  Copies of the copyrighted script for each workshop participant
>  A suggested outline for the speaker/storyteller and closing speech
>  An official CHARLES' CHOICE or Charly's Chioce original poster for each         school or communtiy group
>  A "RECOGNITION CERTIFICATE" for all participant's involvement in the       workshop, rehearsals and presentation of the play and event

ARRIVAL
Expected arriveal is 20 to 30 minutes before the Student Performance                Workshop

FEE
> The standard per school or community fee is $450.00 and includes use of the       school or community space
Half of the fee is payable with the booking of the Student Performance                Workshop.  Payments can be made by cheque with a signed copy of the contract by regular mail or by a PayPal Email Invoice and regular mailed copy of the signed contract.  Bookings are not confirmed until receipt of payment
>  The remaining half of the fee is payable directly to the Facilitator on the day of the Student Performance Workshop (unless other arrangements are made via a contract addendum)
>  The fee also includes additional written scenes (up to 2) for the specific demo and psychographics of a contracted school, school board or community
>  The fee also provides the rights to the school/community group to perform the play in another year, without a Student Performance Workshop facilitator, by the payment of a licensing fee of $35.00 per event or day if performed for several classes in one day or week.  Also, for each Student Performance Workshop and Facilitator booked after the first year, a school/community will receive a 25% discount on the booking year's fee rate
>  Cheques are to be made payable to "Kaitlin Ann Webber/Originals for CC" and are mailed, along with all other documents as above, to 68 Wheatfield Rd. Toronto, ON  M8V 2P6 OR Booking payments and final payments may be made by PayPal Invoice Payments)

BOOKING CHANGES and CANCELLATION
Our best will be done to accomodate changes, but this is a first come, first serve basis.  Booking fees already paid may be applied to one of the next two semesters
A cancellation fee of 30% of the agreed fee will apply if the workshop is cancelled less than two weeks of the workshop date.  If the workshop is cancelled due to uncontrollable circumstances, such as fire, flood, etc., this clause will of course be waived
