As I recall, there are some exceptions to this. One example was where a wife divorced her husband for praticing homosexuality or bestiality. At one point the society only considered adultery as grounds for divorce. Later they included the two mentioned sexual pratices. Spouses who were DFd under the previous practice were allowed to apply for reinstatement, regardless of how recent the DFing occurred.
They may be other exceptions, but I don't know them if there are. Does anyone else know of any?
