Many of the prophecies in the bible are of a dual nature, meaning they have a contemporary and a latter-day fulfilment. I believe that this is one of those. The prophecy is fulfilled twice. Once with the early church, and again with the latter-day church.
I think world in both instances means inhabited or known world. The gospel that will be preached is the redemption of Christ, the kingdom of the gospel is simply the preaching of the reality of Christ's church upon the earth, since the Church is His Kingdom. The message will be a witness against the world, since those who hear it will either accept the message or reject it, and thus be left without excuse at judgment; their treatment of the message being the witness against them.
