Ones the procedure is schedules, we suggest you:
Make sure all your travel documents and medical records are in order, including;
A Passport, Medical travel Visa and other photo identification.
Full Medical records (translated into English). Include all test reports and surgery reports, treatment records, and laboratory results.
Send by email or by postal copy of recent clinical observations / diagnosis / medical report translated into English.
Your Home around the world...
Forms and Medical Questionnaires will be send via fax or email upon request.
All films / CD (for example, X-Rays, MRI's, CT scans, Angiogram) along with relevant reports translated into English.
Glass pathology slides if applicable, along with the original report and a copy of the pathology report translated into English.
