Horizon Family Pass Benefit
Dear Brothers & Sisters:

This page was established to help explain the Horizon Air Family Pass benefit that you now enjoy. In order to help you with this great benefit, I've posted here the eligibility rules, the cost, and the procedure that will get you an Horizon Pass.

I've also posted an Horizon Route Map that you can click on and download. I will need some "front time" to help process your individual requests. Even though the passes aren't dated, I will still need time to purchase them and mail them to you.

Family Pass
Family passes allow your extended family members to travel on Horizon. Please inform them of the following items.

Requirements: Employee must be an active regular-status employee, have been with the company a year, and have a current Family Pass Census Card on file in the Seattle Pass Bureau.

Eligibility: Brothers (includes half-brothers) and their spouses, sisters (includes half-sisters) and their spouses, grandparents, and mother-in-law/father-in-law/step parent-in-laws. Only one set of in-laws may be eligible per calendar year. Step parents/natural parents without current benefits, and children (natural/step) not enrolled in school who are over the age of 19. Single employees may have one employee designated guest (see above).

To obtain tickets: (John does this part.) Send a completed HA-141 along with a check made out to Horizon Air for $21 roundtrip per person plus applicable segment taxes, which vary (for the exact total amount, contact the Pass Bureau). Withholding taxes (about $6) based on the taxable value of the family pass (currently $21) will be deducted from your paycheck by Payroll. The check must be from the employee. No business checks or credit cards will be accepted. Allow three to five days for processing in Portland and Seattle. If you wish to receive your tickets faster, please send a self-addressed, stamped envelope.

Rules: Tickets are nonrefundable and nontransferable. Tickets do not expire, but they do become invalid upon the employee's termination. They must be used for cities issued. Travel is valid on Horizon only. Horizon is not responsible for lost/stolen passes or checks.

Nonrevenue do's and don'ts
Nonrevenue travelers must:

Always be considerate of fare-paying customers, who make all privileges possible. Every effort will be made to board all nonrevenue travelers, but paying customers will always be given first priority and attention.
Always cooperate with airline personnel.
Accept quietly and graciously inconveniences such as flight delays, being asked to relinquish a seat or being denied a meal (nonrev travelers should discreetly inform the flight attendant of their status to ensure they are served only if there are enough meals for all other passengers on board). Complaints of unfair treatment should only be made to the traveling employee�s immediate supervisor or the Pass Bureau.
Only pre-board when invited to by the gate agent and otherwise board toward the end of the paying customers' line.
Give the flight attendant their full attention during the safety briefing, as a courtesy to the crew and an example to others on board.
Leave their seating area free of newspapers and trash. This help can be crucial in assisting Horizon with on-time performance, and it is always appreciated by fellow employees.
