Financial Policy
- Until your insurance coverage and deductible can be confirmed by our staff, you are considered a cash patient. We accept cash, checks, Visa, and MasterCard.

- This office may make payment plan arrangements on an individual basis. We will only bill your Primary Insurance carrier. If you need documentation to bill a secondary carrier, the front desk will provide you a ledger of services performed.

- In the event your insurance company does not cover the cost of your treatment, any balance is your responsibility and is not eligible for discounts. It is your responsibility to verify your benefits for in/out of network services. If you ahve any questions about what your insurance company tells you, the front desk staff will be happy to help you with understanding the terminology.

- You are responsible directly to Natural Family Health Care for payment of services. Natural Family Health Care does not accept responsibility for disputing a denied or non-paying claim. In the event of a denied or non-paying claim, your payment is due to NFHC within 30 days of notification from your insurance company.

- Co-payment is due on the day of treatment for services rendered.

- If your insurance company accidentally sends you a check for our services, it is your responsibility to bring or mail it to our office within 10 days.

- If it becomes necessary to effect collections of an amount due, the undersigned agrees to pay for all cost and expense incurred in the collection process.

- There is a $30.00 fee for a missed appointment or cancelling your appointment after 8:30 a.m. the day of your appointment. You may leave a voicemail to cancel your appointment if we cannot answer the phone.

- There will be a finance charge of 10% added to all accounts that carry a balance for more than 30 days after insurance pays and an initial statement has been sent. If your statement is for a large amount and payment arrangements have been made with the front desk within 30 days, then this fee will be waived.

- There will be a $20 copy fee for records for personal use. If sent to a physician, this is waived.
