| ============================================================================ Patient Log ok, call me a dork. i never liked the handwritten ct patient logs. this log may not work well with busy hospitals, but for the little imaging center i work for, i find it easier to enter my patient information this way. when you click on the link above, it will open the spread sheet (Microcoft Excel). just save it to your computer, and go from there. if you have any questions about it, send me an email. there are 4 page tabs. the last page is the monthly and year to date summary. the only data you need to enter is the prior month's information. everything else is on the first 2 tabs. don't bother messing with the "calculations" tab, that was just a place for me to work out some formulas. fill in the blank portions on the first tab. under the scan information, use "1" for one exam, or "2" for an exam that was with and without contrast. the second tab is for eGFR. i only enter data here if it is requred. our policy is patients over 60 get labs, so that's what it's set for. ============================================================================ GE QA form all you people with the GE VCT 64 slice scanner... i've made this spreadsheet for ya. i use this every day when i do my morning qa's. i've added some things to mine so that i can track my cals and contrast warmer. it's designed for my imaging center (monday through friday), but can be changed easily for a 30 day report. i went through the technical books that came with our GE 64 VCT, and wrote 2 qa protocols. i've also sat down with our phycisist to make sure everything is ACR compliant. if you have any questions or suggestions, drop me an email. ============================================================================ QA Mammo Reject Analysis mammo lead techs and/or managers should like this one. our office has 3 mammo rooms. our lead tech was trying to calculate by hand all the reject mammo films. i told her to let me give it a try. this is what i came up with. |
