Cytomegalovirus Pap staining, owl´s eye binuclated cell

The presence of many cells with intranuclear huge pink inclusions surrounded by a clear halo ("owl's eye" appearance)is highly suggestive of CMV infection. The nuclei in these cells are several times the size of nuclei in neighboring pulmonary alveolar macrophages. The patient was in a rapidly downhill course, and despite foscarnet and ganciclovir therapy, died three weeks later. Autopsy revealed multisystemic Cytomegalovirus infection, including extensive bilateral interstitial pneumonia, gastritis, colitis, hepatitis, and encephalitis.
