The tumor cells bear some resemblance to normal hepatocytes with granular cytoplasm, distinct cell membranes, and rounded nuclei, but are additionally distinguished by the presence of cytologic atypias. These atypias are characterized by varying degrees of nuclear enlargement and hyperchromasia with clumping and irregularity of the chromatin and asymmetric nuclear contours; prominent nucleoli are frequent; the nuclear-cytoplasmic ratio is increased. Striking pleomorphism, bizarre mitotic figures, and tumor giant cells can be seen. Alpha-fetoprotein immunoreactivity was strongly positive ( and the serum level was 1,260 units). Besides AIDS, the patient has also Chronic Hepatitis C. (There are data suggesting an effect of HCV infection on HCC risk among adults with AIDS.)
