Ascites fluid: high grade MALT lymphoma

Drs. Prolla and Diehl's INTERESTING CASE OF THE MONTH March 2006 case C

Gastric high grade MALT lymphoma 200x Pap staining, great number of abnormal lymphoid cells

Gastric high grade MALT lymphoma 500x Pap staining, high power view of lymphoblasts: coarse chromatin

Gastric high grade MALT lymphoma 500x M-G-G staining, notice basophilic cytoplasm with small lipid vacuoles

Gastric high grade MALT lymphoma 500x M-G-G staining, notice basophilic cytoplasm with small lipid vacuoles

Gastric high grade MALT lymphoma 500x M-G-G staining, notice basophilic cytoplasm with small lipid vacuoles

The patient had a known gastric high grade B-cell lymphoma (MALT type). He developed ascites, and the sediment had 99% lymphoblasts, with fat vacuoles and coarse chromatin content. The immunophenotyping confirmed the B-cell origin and its monoclonality. I didn't find any other documented case of high grade gastric MALToma causing ascites, in the cytologic literature.
