A goodly word (kalimatan tayyibatan) as a goodly tree, whose root is firmly fixed, and its branches (reach) to the sky (i.e. very high). (  سورة إبراهيم  , Ibrahim, Chapter #14, Verse #24)”. Mention of kalema is also found at (  سورة فاطر  , Fatir, Chapter #35, Verse #10)]

Do you really believe this and follow?

Who believe in the Ghaib and perform As-Salat (Iqamat-as-Salat), and spend out of what We have provided for them [i.e. give Zakat , spend on themselves, their parents, their children, their wives, etc., and also give charity to the poor and also in Allah's Cause - Jihad]. (  سورة البقرة  , Al-Baqara, Chapter #2, Verse #3).

Do you perform regular prayers?

O you who believe! Observing As-Saum (the fasting) is prescribed for you as it was prescribed for those before you, that you may become Al-Muttaqun (the pious - See V.2: 2). (  سورة البقرة  , Al-Baqara, Chapter #2, Verse #183).

Do you fast in the month of Ramadan?

As-Sadaqat (here it means zakat ) are only for the Fuqara' (poor), and Al-Masakin (the poor) and those employed to collect (the funds), and to attract the hearts of those who have been inclined (towards Islam), and to free the captives, and for those in debt, and for Allah's Cause (i.e. for Mujahidun - those fighting in a holy battle), and for the wayfarer (a traveller who is cut off from everything); a duty imposed by Allah. And Allah is All-Knower, All-Wise. (  سورة التوبة  , At-Taubah, Chapter #9, Verse #60).

Do you pay 2.5% of your saved annual income as compulsory charity (zakat)?

In it are manifest signs (for example), the Maqam (place) of Ibrahim (Abraham); whosoever enters it, he attains security. And Hajj (pilgrimage to Makkah) to the House (Ka'bah) is a duty that mankind owes to Allah, those who can afford the expenses (for one's conveyance, provision and residence); and whoever disbelieves [i.e. denies Hajj (pilgrimage to Makkah), then he is a disbeliever of Allah], then Allah stands not in need of any of the 'Alamin (mankind, jinn and all that exists) . (  سورة آل عمران  , Aal-e-Imran, Chapter #3, Verse #97).

Did you perform or plan to perform your Hajj?

O you who believe! Avoid much suspicion; indeed some suspicions are sins. And spy not, neither backbite one another. Would one of you like to eat the flesh of his dead brother? You would hate it (so hate backbiting) . And fear Allah. Verily, Allah is the One Who forgives and accepts repentance, Most Merciful. (  سورة الحجرات  , Al-Hujraat, Chapter #49, Verse #12).

Are you aware of the sins of talking behind?

And spend (in charity) of that with which We have provided you before death comes to one of you, and he says: "My Lord! If only You would give me respite for a little while (i.e. return to the worldly life), then I should give Sadaqah (i.e. Zakat ) of my wealth, and be among the righteous [i.e. perform Hajj (pilgrimage to Makkah) and other good deeds]. (  سورة المنافقون  , Al-Munafiqoon, Chapter #63, Verse #10).

(Resurrection, recompense of their good and bad deeds, Paradise and Hell). (  سورة البقرة  , Al-Baqara, Chapter #2, Verse #4).

Do you believe in the life hereafter?

Verily, in this is an Ayah (proof or sign), yet most of them (polytheists, pagans who do not believe in Resurrection) are not believers. (  سورة الشعراء  , Ash-Shuara, Chapter #26, Verse #8).

Do you believe in the Day of Resurrection?

Look then at the effects (results) of Allah's Mercy: how He revives the earth after its death. Verily, that (Allah) (Who revived the earth after its death) shall indeed raise the dead (on the Day of Resurrection), and He is Able to do all things. (  سورة الروم  , Ar-Room, Chapter #30, Verse #50).

Do you believe that you will be raised from the dead again?

And (remember) when We said to the angels: "Prostrate yourselves unto Adam." So they prostrated themselves except Iblis (Satan). He was one of the jinn; he disobeyed the Command of his Lord. Will you then take him (Iblis) and his offspring as protectors and helpers rather than Me while they are enemies to you? What an evil is the exchange for the Zalimun (polytheists, and wrong-doers). (  سورة الكهف  , Al-Kahf, Chapter #18, Verse #50).

Do you belive in the angels and the jinns (i.e.the invisible world around us)?

Say (O Muhammad صلى الله عليه وسلم to mankind): "If you (really) love Allah then follow me (i.e. accept Islamic Monotheism, follow the Qur'an and the Sunnah), Allah will love you and forgive you your sins. And Allah is Oft-Forgiving, Most Merciful." (  سورة آل عمران  , Aal-e-Imran, Chapter #3, Verse #31).

Do you follow the Prophet’s (s) sunnah to please Allah?

Verily, Allah enjoins Al-'Adl (i.e. justice and worshipping none but Allah Alone - Islamic Monotheism) and Al-Ihsan [i.e. to be patient in performing your duties to Allah, totally for Allah's sake and in accordance with the Sunnah (legal ways) of the Prophet صلى الله عليه وسلم in a perfect manner], and giving (help) to kith and kin (i.e. all that Allah has ordered you to give them e.g., wealth, visiting, looking after them, or any other kind of help), and forbids Al-Fahsha' (i.e all evil deeds, e.g. illegal sexual acts, disobedience of parents, polytheism, to tell lies, to give false witness, to kill a life without right), and Al-Munkar (i.e all that is prohibited by Islamic law: polytheism of every kind, disbelief and every kind of evil deeds), and Al-Baghy (i.e. all kinds of oppression). He admonishes you, that you may take heed. (  سورة النحل  , An-Nahl, Chapter #16, Verse #90).

Are you one of those who do not tell lies or give false statement?

Shaitan (Satan) threatens you with poverty and orders you to commit Fahsha (evil deeds, illegal sexual intercourse, sins); whereas Allah promises you Forgiveness from Himself and Bounty, and Allah is All-Sufficient for His creatures' needs, All-Knower. (  سورة البقرة  , Al-Baqara, Chapter #2, Verse #268).

Are you aware of the powers of your avowed enemy (i.e., Satan)?

And if an evil whisper comes to you from Shaitan (Satan), then seek refuge with Allah. Verily, He is All-Hearer, All-Knower. (  سورة الأعراف  , Al-Araf, Chapter #7, Verse #200).

Do you know that Allah is the only one who could protect you from the deceptions of Satan?

Say (O Muhammad صلى الله عليه وسلم): "Come, I will recite what your Lord has prohibited you from: Join not anything in worship with Him; be good and dutiful to your parents; kill not your children because of poverty - We provide sustenance for you and for them; come not near to Al-Fawahish (shameful sins, illegal sexual intercourse) whether committed openly or secretly; and kill not anyone whom Allah has forbidden, except for a just cause (according to Islamic law). This He has commanded you that you may understand. (  سورة الأنعام  , Al-Anaam, Chapter #6, Verse #151).

Do you believe that Allah is the only provider of our sustenance?

But he who is greedy miser and thinks himself self-sufficient(  سورة الليل  , Al-Lail, Chapter #92, Verse #8).

Are you aware that we could never be self-sufficient without Allah’s help?

O you who believe! Why do you say that which you do not do(  سورة الصف  , As-Saff, Chapter #61, Verse #2).

Do you follow what you ask others to do?

The Messenger (Muhammad صلى الله عليه وسلم) believes in what has been sent down to him from his Lord, and (so do) the believers. Each one believes in Allah, His Angels, His Books, and His Messengers. (They say), "we make no distinction between one another of His Messengers" - and they say, "we hear, and we obey. (we seek) Your Forgiveness, our Lord, and to You is the return (of all)." (  سورة البقرة  , Al-Baqara, Chapter #2, Verse #285).

Are you a believer who obeys the Quran and authentic Sunnah without any questioning?

And most of them believe not in Allah except that they attribute partners unto Him [i.e. they are Mushrikun i.e. polytheists. See Verse 6: 121]. (  سورة يوسف  , Yusuf, Chapter #12, Verse #106).

Are you aware of different kinds of shirk that people fall into knowingly and unknowingly?

Say (O Muhammad صلى الله عليه وسلم): "I am only a man like you. It has been revealed to me that your Ilah (God) is One Ilah (God - i.e. Allah). So whoever hopes for the Meeting with his Lord, let him work righteousness and associate none as a partner in the worship of his Lord." (  سورة الكهف  , Al-Kahf, Chapter #18, Verse #110)

Do you know that shirk (association of partner with Allah) is an unforgivable sin?

Invite (mankind, O Muhammad صلى الله عليه و سلم) to the Way of your Lord (i.e. Islam) with wisdom (i.e. with the Divine Revelation and the Qur'an) and fair preaching, and argue with them in a way that is better. Truly, your Lord knows best who has gone astray from His Path, and He is the Best Aware of those who are guided. (  سورة النحل  , An-Nahl, Chapter #16, Verse #125)

Do you invite people (Dawah) in and around your circle (family and friends) to Islam?

Do you agree that learning Arabic is compulsory in order to fully understand the Quran?

And tell the believing women to lower their gaze (from looking at forbidden things), and protect their private parts (from illegal sexual acts) and not to show off their adornment except only that which is apparent (like both eyes for necessity to see the way, or outer palms of hands or one eye or dress like veil, gloves, head-cover, apron, etc.), and to draw their veils all over Juyubihinna (i.e. their bodies, faces, necks and bosoms) and not to reveal their adornment except to their husbands, or their fathers, or their husband's fathers, or their sons, or their husband's sons, or their brothers or their brother's sons, or their sister's sons, or their (Muslim) women (i.e. their sisters in Islam), or the (female) slaves whom their right hands possess, or old male servants who lack vigour, or small children who have no sense of feminine sex. And let them not stamp their feet so as to reveal what they hide of their adornment. And all of you beg Allah to forgive you all, O believers, that you may be successful. (  سورة النور  , An-Noor, Chapter #24, Verse #31)

If you are woman, do you obey the orders on hijab and veiling? And if you are a father of a daughter or a husband, do you ensure that your womanfolks are observing it?

He [Harun (Aaron)] said: "O son of my mother! Seize (me) not by my beard, nor by my head! Verily, I feared lest you should say: 'You have caused a division among the Children of Israel, and you have not respected my word!' " (  سورة طه  , Taha, Chapter #20, Verse #94)

Growing and keeping of beard is a sunnah of all prophets. Do you follow this sunnah?

For parents, a sixth share of inheritance to each if the deceased left children; if no children, and the parents are the (only) heirs, the mother has a third; if the deceased left brothers (or sisters), the mother has a sixth. (The distribution in all cases is) after the payment of legacies he may have bequeathed or debts. You know not which of them, whether your parents or your children, are nearest to you in benefit; (these fixed shares) are ordained by Allah. And Allah is Ever All-Knower, All-Wise. (  سورة النساء  , An-Nisa, Chapter #4, Verse #11)

Have you paid your obligatory bridal money to your wife at the time of marriage? If you are unmarried, are you aware of it?

Have you seen him who takes his own lust (vain desires) as his ilah (god)? And Allah knowing (him as such), left him astray, and sealed his hearing and his heart, and put a cover on his sight. Who then will guide him after Allah? Will you not then remember? (  سورة الجاثية  , Al-Jathiya, Chapter #45, Verse #23)

Do you give precedence of your own choices over the choices of Allah and His Messenger?

It is not for a believer, man or woman, when Allah and His Messenger have decreed a matter that they should have any option in their decision. And whoever disobeys Allah and His Messenger, he has indeed strayed into a plain error. (  سورة الأحزاب  , Al-Ahzab, Chapter #33, Verse #36)

Verily, the hypocrites seek to deceive Allah, but it is He Who deceives them. And when they stand up for As-Salat (the prayer), they stand with laziness and to be seen of men, and they do not remember Allah but little. (  سورة النساء  , An-Nisa, Chapter #4, Verse #142)

Do you give full concentration in your prayers?

The good deed and the evil deed cannot be equal. Repel (the evil) with one which is better (i.e. Allah orders the faithful believers to be patient at the time of anger, and to excuse those who treat them badly) then verily he, between whom and you there was enmity, (will become) as though he was a close friend. (  سورة فصلت  , Fussilat, Chapter #41, Verse #34)

Is your faith strong enough to repel evil when faced with one?

Shaitan (Satan) wants only to excite enmity and hatred between you with intoxicants (alcoholic drinks) and gambling, and hinder you from the remembrance of Allah and from As-Salat (the prayer). So, will you not then abstain? (  سورة المائدة  , Al-Maeda, Chapter #5, Verse #91)

Do you hate and stay away from drinking alcohol and gambling?

O you who believe! Verily, there are many of the (Jewish) rabbis and the (Christian) monks who devour the wealth of mankind in falsehood, and hinder (them) from the Way of Allah (i.e. Allah's religion of Islamic Monotheism). And those who hoard up gold and silver [Al-Kanz: the money, the Zakat of which has not been paid] and spend them not in the Way of Allah, announce unto them a painful torment. (  سورة التوبة  , At-Taubah, Chapter #9, Verse #34)

Do you see a similarity of today’s grave worshippers among us who takes the dead as their intercessors? Are you one of them?

O you who believe! Enter perfectly in Islam (by obeying all the rules and regulations of the Islamic religion) and follow not the footsteps of Shaitan (Satan). Verily! He is to you a plain enemy. (  سورة البقرة  , Al-Baqara, Chapter #2, Verse #208)

Do you avoid blindly following the disbelievers (i.e., Jews, Christians, Buddhists and Hindus)?

Those who eat Riba (usury) will not stand (on the Day of Resurrection) except like the standing of a person beaten by Shaitan (Satan) leading him to insanity. That is because they say: "Trading is only like Riba (usury)," whereas Allah has permitted trading and forbidden Riba (usury). So whosoever receives an admonition from his Lord and stops eating Riba (usury) shall not be punished for the past; his case is for Allah (to judge); but whoever returns [to Riba (usury)], such are the dwellers of the Fire - they will abide therein. (  سورة البقرة  , Al-Baqara, Chapter #2, Verse #275)

Do you avoid bank interests on your financial plannings?

And We have enjoined on man to be dutiful and kind to his parents. His mother bears him with hardship. Grant me the power and ability that I may be grateful for Your Favour which You have bestowed upon me and upon my parents, and that I may do righteous good deeds, such as please You, and make my offspring good. Truly, I have turned to You in repentance, and truly, I am one of the Muslims (submitting to Your Will)." (  سورة الأحقاف  , Al-Ahqaf, Chapter #46, Verse #15)

Then, there has succeeded them a posterity who have given up As-Salat (the prayers) [i.e. made their Salat (prayers) to be lost, either by not offering them or by not offering them perfectly or by not offering them in their proper fixed times] and have followed lusts. So they will be thrown in Hell. (  سورة مريم  , Maryam, Chapter #19, Verse #59)

Do you offer your regular prayers on time?

They ask you (O Muhammad صلى الله عليه وسلم) what is lawful for them (as food). Say: "Lawful unto you are At-Tayyibat [all kind of halal (lawful-good) foods which Allah has made lawful (meat of slaughtered eatable animals, milk products, fats, vegetables and fruits)]. And those beasts and birds of prey which you have trained as hounds, training and teaching them (to catch) in the manner as directed to you by Allah; so eat of what they catch for you, but pronounce the Name of Allah over it, and fear Allah. Verily, Allah is Swift in reckoning." (  سورة المائدة  , Al-Maeda, Chapter #5, Verse #4)

Do you care about haram (prohibited) and halal (allowed) on your daily diets?

And obey not the disbelievers and the hypocrites, and harm them not (in revenge for their harming you till you are ordered). And put your trust in Allah, and Sufficient is Allah as a Wakil (Trustee, or Disposer of affairs). [Tafsir Al-Qurtubi] (  سورة الأحزاب  , Al-Ahzab, Chapter #33, Verse #48)

Are you truthful?

And verily, among them is a party who distort the Book with their tongues (as they read), so that you may think it is from the Book, but it is not from the Book, and they say: "This is from Allah," but it is not from Allah; and they speak a lie against Allah while they know it. (  سورة آل عمران  , Aal-e-Imran, Chapter #3, Verse #78)

Do you transmit truth without any distortion even while on gossips, idle talks and jokes?

Do you strictly follow sunnah unlike the majority rule following (i.e., Democracy, Madhabs, Sects, Imams, Scholars, etc.)?

And do not marry Al-Mushrikat (idolatresses, etc.) till they believe (worship Allah Alone). And indeed a slave woman who believes is better than a (free) Mushrikah (idolatress), even though she pleases you. And give not (your daughters) in marriage to Al-Mushrikun till they believe (in Allah Alone) and verily, a believing slave is better than a (free) Mushrik (idolater), even though he pleases you. Those (Al-Mushrikun) invite you to the Fire, but Allah invites (you) to Paradise and Forgiveness by His Leave, and makes His Ayat (proofs, evidence, verses, lessons, signs, revelations, etc.) clear to mankind that they may remember. (  سورة البقرة  , Al-Baqara, Chapter #2, Verse #221)

Do you dispprove of inter-faith marriages between Muslims and Pagans (idol worshippers)?

And (remember) when Ibrahim (Abraham) said to his father Azar: "Do you take idols as alihah (gods)? Verily, I see you and your people in manifest error. " (  سورة الأنعام  , Al-Anaam, Chapter #6, Verse #74)

Do you avoid nature worshipping like the pagans celebrating and observing rituals and festivals of local non-Islamic culture and customs?

O you who believe! Take not the Jews and the Christians as Auliya' (friends, protectors, helpers), they are but Auliya' of each other. And if any amongst you takes them (as Auliya'), then surely he is one of them. Verily, Allah guides not those people who are the Zalimun (polytheists and wrong-doers and unjust). (  سورة المائدة  , Al-Maeda, Chapter #5, Verse #51)

Do you aviod the religious occasions of other religions (i.e., Judaism, Christianity, Buddhism, Hinduism, etc.)?

Jihad (holy fighting in Allah's cause) is ordained for you (Muslims) though you dislike it, and it may be that you dislike a thing which is good for you and that you like a thing which is bad for you. Allah knows but you do not know. (  سورة البقرة  , Al-Baqara, Chapter #2, Verse #216)

Are you ready for jihad only for the pleasure of Allah?

O you who believe! Fear Allah (by doing all that He has ordered and by abstaining from all that He has forbidden) as He should be feared. [Obey Him, be thankful to Him, and remember Him always], and die not except in a state of Islam [as Muslims (with complete submission to Allah)].
(  سورة آل عمران  , Aal-e-Imran, Chapter #3, Verse #102)

We make no distinction between any of them, and to Him we have submitted (in Islam)." (  سورة البقرة  , Al-Baqara, Chapter #2, Verse #136)

Do you stay away from the debate and arguments of exalting Prophet Muhammad (s) over other Prophets?

O you who believe (Muslims)! When the call is proclaimed for the Salat (prayer) on Friday (Jumu'ah prayer), come to the remembrance of Allah [Jumu'ah religious talk (Khutbah) and Salat (prayer)] and leave off business (and every other thing). That is better for you if you did but know! (  سورة الجمعة  , Al-Jumua, Chapter #62, Verse #9)

Do you regularly attend the Friday congregational Jumma prayer?

O you who believe! Ward off yourselves and your families against a Fire (Hell) whose fuel is men and stones, over which are (appointed) angels stern (and) severe, who disobey not, (from executing) the Commands they receive from Allah, but do that which they are commanded.
(  سورة التحريم  , At-Tahrim, Chapter #66, Verse #6)

O you who believe! Turn to Allah with sincere repentance! Their Light will run forward before them and (with their Records - Books of deeds) in their right hands. They will say: "Our Lord! Keep perfect our Light for us [and do not put it off till we cross over the Sirat (a slippery bridge over the Hell) safely] and grant us forgiveness. Verily, You are Able to do all things." (  سورة التحريم  , At-Tahrim, Chapter #66, Verse #8)

Are you one of those who sincerely repent as soon as the realization comes that a mistake has been made?

Not equal are those of the believers who sit (at home), except those who are disabled (by injury or are blind or lame), and those who strive hard and fight in the Cause of Allah with their wealth and their lives. Allah has preferred in grades those who strive hard and fight with their wealth and their lives above those who sit (at home). Unto each, Allah has promised good (Paradise), but Allah has preferred those who strive hard and fight, above those who sit (at home) by a huge reward. (  سورة النساء  , An-Nisa, Chapter #4, Verse #95)

Are you an active Muslim striving for Allah’s causes?

Total score

Note: This is not all. There are many more do’s and don’ts for a Muslim in the Qur’an. I have just quoted some major points instead of copying everything. It is the responsibility of a Muslim to inquire, learn and adopt all the commands of Allah and His Prophet (s).

Conclusion: Higher the score you have earned based on your self assessment signifies how good of a Muslim you are. If your score is high you are definitely a good Muslim. And if your score is low, then you need to work on your weaknesses to overcome them by reading the Quran and authentic Sunnah (i.e., the ahadiths). Your score would definitely improve if and when you would take the same test again after some soul searching and studies.

"But if you turn away [from accepting my doctrine of Islamic Monotheism, i.e. to worship none but Allah], then no reward have I asked of you; my reward is only from Allah, and I have been commanded to be of the Muslims (i.e. those who submit to Allah's Will)." (  سورة يونس  , Yunus, Chapter #10, Verse #72)
