TRIP PERMISSON FORM

I assume full responsibility for the safety and conduct of my
son/daughter _________________while on a trip
to Mitsuwa on Nov 22nd. I understand that the group is going
by bus, will leave school at 3:30 return about 6:30 and
that they will be chaperoned by at least one member of this faculty.
I also understand that it is the student's responsibility to make up
any work missed by going on this trip; that no alcohol is to be taken
or consumed on the trip; and, that the student must remain with the
group at all times.
I have no objection to emergency first aid care in the event of an accident.
